Vertigo is not itself a disease, but rather a symptom of various syndromes and disorders that jeopardize balance function, which is essential for daily activities. It is an abnormal sensation of motion that usually occurs in the absence of motion, or when a motion is sensed inaccurately. Due to the complexity of the etiopathogenesis of vertigo, many pharmacological treatments have been tested for efficacy on vertigo. Studio prospettico ideato per la valutazione delle differenze tra la conversione canalare o il rientro degli otoliti nei canali semicircolari successivo alle manovre terapeutiche nei pazienti affetti da VPPB. Sono stati valutati 196 pazienti affetti da VPPB, 127 dei quali corrispondevano ai criteri di inclusione. L’età media dei pazienti era di 54.74 anni. Il canale orizzontale è stato coinvolto in 30 casi e il canale posteriore in 97 pazienti. I pazienti con sordità neurosensoriale presentavano forme ricorrenti di VPPB, rispetto a quelli con udito normale. Two hundred and two consecutive patients with dizziness were evaluated using blind neuro-otological testing and examination, blind psychiatric examination, including structured interviews (according to DSM-IV), the Symptom Check-List (SCL 90 R), and the State-Trait Anxiety Inventory (STAI). In 28% of the patients (N=50) dizziness was of organic origin (O group); in 55.3% (N=99) of psychogenic origin (P group) and in 16.8% comorbid psychiatric disorders were found (Mixed group). Seventy-eight consecutive subjects, 43 males and 35 females, ranging from 43 to 87 years of age suffering from dizziness and/or tinnitus due to vertebrobasilar insufficiency without other central nervous system diseases were treated with fixed combination of cinnarizine and dimenhydrinate two times a day and for two months. Data were collected considering the following parameters: age, sex, vestibular symptoms (unsteadiness, staggering, tendency to fall, swaying, vertigo due to change of position, bowing, walking, eye movements), headache, tinnitus, impaired hearing and aural fullness.
